Ո՞ւմ է խորհուրդ տրվում ստուգել «քաղցկեղային գեների» առկայությունը

Կանխարգելիչ մաստէկտոմիան, որին վերջերս գնաց հանրահայտ դերասանուհի Անջելինա Ջոլին, գրավեց շատերի ուշադրությունը . դերասանուհին որոշել էր վիրահատվել այն բանից հետո, երբ իմացել էր, որ այսպես կոչված «քաղցկեղային» գեների կրող է, ինչը զգալիորեն բարձրացնում է կրծքագեղձի ու ձվարանների քաղցկեղով հիվանդանալու հավանականությունը:
Կանայք բավականին հեշտությամբ կարող են պարզել արդյոք իրենք BRCA1 եւ BRCA2 քաղցկեղային գեների կրող են, թե ոչ. նման անալիզներ արվում են նաեւ Հայաստանում: Սակայն ում է խորհուրդ տրվում ստուգվել, իսկ ում ՝ ոչ:
Ինչպես NEWS.am Medicine-ի թղթակցին հայտնել է նշանավոր ուռուցքաբան եւ Վիեննայի բժշկական համալսարանի պրոֆեսոր Գյունտեր Գ. Շտեգերը, անհրաժեշտություն չկա, որ բոլոր կանայք անխտիր վազեն այդ անալիզները հանձնելու. քաղցկեղային գեներ կան կանանց ընդամենը 3-4 տոկոսի մոտ: Ստուգվել արժե միայն նրանց, ում ազգականների շրջանում կան երկու եւ ավելի կանայք ՝ կրծքագեղձի կամ ձվարանների քաղցկեղով (հատկապես եթե նրանք հիվանդացել են երիտասարդ տարիքում), կամ էլ տղամարդիկ շագանակագեղձի քաղցկեղով: Իսկ եթե ընտանեկան անամնեզում նման հիվանդություններ չկան, ըստ էության ստուգվելն իմաստ չունի:
Անհրաժեշտ է արդյո՞ք կանխարգելիչ մաստէկտոմիան (կրծքագեղձի կանխարգելիչ հեռացում), եթե կանայք հայտնաբերում են գեներ, որոնք բարձրացնում են քաղցկեղի առաջացման վտանգը:
Այս հարցում մասնագետների տեսակետները ոչ միշտ են համընկնում ՝ ոմանք հավաստիացնում են, որ վիրահատությունը պարտադիր է, մյուսները պնդում են, որ գեների առկայությունը ամենեւին էլ չի նշանակում, որ այդ կանայք պարտադիր պետք է հիվանդանան քաղցկեղով: Բժիշկ Շտեգերը համարում է, որ նույնիսկ եթե գենը հայտնաբերված է, վիրահատությունը ոչ միշտ է միանշանակ ցուցված: Սակայն պետք է անպայման բուժող բժշկի հետ քննարկել վիրահատության հնարավորությունն ու նպատակահարմարությունը, հասկանալ բոլոր թերություններն ու առավելությունները, հաշվի առնել հիվանդ կնոջ կարիքները ու միայն դրանից հետո որոշում կայացնել:
