Շագանակագեղձի քաղցկեղը ամենատարածված քաղցկեղներից մեկն է: Այն կազմում է տղամարդկանց մոտ ախտորոշված բոլոր չարորակ ուռուցքների ավելի քան 14%-ը: Հիվանդությունը դանդաղ է զարգանում և կարող է տարիներ շարունակ զարգանալ առանց հատուկ ախտանշանների: Հիվանդության և դրա առաջացմանը նպաստող գործոնների մասին խոսել ենք  «Աստղիկ» ԲԿ-ի ուրոլոգ Արամ Ադամյանի հետ։

-Վաղ փուլերում շագանակագեղձի ուռուցքը չի անցնում շագանակագեղձի սահմաններից և գործնականում ոչ մի կերպ չի արտահայտվում։ Ինչպե՞ս կարելի է այս դեպքում ախտորոշել հիվանդությունը։

-Խոսքս պետք է սկսեմ հետևյալով` հիվանդությունը ավելի հեշտ է կանխարգելել, քան բուժել: Առնվազն ավելի հեշտ է կատարել սկրինինգ հետազոտություններ, քան պայքարել հիվանդությունների՝ մասնավորապես քաղցկեղներիի տարածված ձևերի դեմ:

Շագանակագեղձի քաղցկեղը երկրորդ ամենատարածված քաղցկեղն է տղամարդկանց մոտ, ախտահարելով մոտ 1,1 մլն տղամարդու 2012թ. ի տվյալներով: Շագանակգեղձի քաղցկեղի սկրինինգային ախտորոշման համար կատարվում է պրստատ սպեցիֆիկ անտիգենի որոշում (PSA)` 50տարեկանից ընդհանուր պոպուլյացիայում, 45տ ից երբ առկա է ընտանեկան անամնեզ կամ, երբ բուժառուն ունի աֆրիկյան ծագում: Կարևոր հետազոտություն է համարվում շագանակագեղձի մատնային զննումը, իսկ գործիքային քննություններից շագանակգեղձի մուլտիպարամետրիկ մագնիտո-ռեզոնանսային հետազոտությունը (mpMRI):

-Ո՞ր գործոններն են նպաստում հիվանդության առաջացմանը և զարգացմանը։

-Շագանակագեղձի ռիսկի գործոններն են համարվում`տարիքը, ժառանգական նախատրամադրվածությունը և աշխարհագրական պատկանելիությունը: Ռիսկի գործոններին են դասվում նաև մետաբոլիկ համախտանիշի/ճարպակալման առկայությունը, ալկոհոլի չարաշահումը, իհարկե անմասն չի մնում ծխելը:

-Կան միջազգային հետազոտություններ, որոնք ցույց են տալիս ծխելու և շագանակագեղձի քաղցկեղի զարգացման միջև հստակ կապի մասին: Ինչպե՞ս կարելի է նվազեցնել հիվանդության առաջացման ռիսկը այս դեպքում։

-Ծխելը կապված է շագանակագեղձի քաղցկեղից բարձր մահացության հետ: Ընդ որում ռիսկը որոշակիորեն կարող է պահպանվել անգամ ծխելը թողնելուց հետո: Կապ ունի, թե բուժառուն որքան ժամանակ է ծխել, և երբ է թողել ծխելը:  Մասնավորապես ըստ որոշ միջազգային հետազոտությունների ծխելը համարվում է շագանակագեղձի քաղցկեղից մահացության ռիսկի նշանակալի գործոն, նաև նշվում է օրական ծխախոտի քանակի օգտագործման և ռիսկի բարձրացման կապը։

Բանն այն է, որ սիգարետի ծուխը պարունակում է 100-ից ավելի վնասակար և քաղցկեղածին նյութեր, որոնք նպաստում են քաղցկեղի առաջացմանը, իսկ հետագայում բացասաբար են ազդում բուժման արդյունավետության վրա։ Այդ իսկ պատճառով կարևոր է, որպեսզի մարդիկ չսկսեն ծխել, իսկ եթե ծխում են վերջ տան այդ վնասակար սովորությանը։ Այն դեպքերում, երբ չի հաջողվում թողնել ծխելը, խորհուրդ է տրվում անցում կատարել նիկոտինի մատակարարման անծուխ այլընտրանքների, ինչպիսիք են նիկոտինային սպեղանիները, մաստակները կամ ծխախոտի տաքացման համակարգերը։ Այս այլընտրանքները ևս ամբողջովին անվնաս չեն, սակայն գիտական հետազոտությունները ցույց են տալիս որ դրանք կարող են նվազեցնել հիվանդացության ռիսկը։
