Բրոնխոսկոպիա. Ինչպես են զննում թոքերը

Բրոնխոսկոպիան ախտորոշիչ հետազոտություն է, որը բժշկին թույլ է տալիս զննել հիվանդի թոքերը և դրանց օդատար խողովակները։

Բրոնխոսկոպիայի ընթացքում բժիշկը բարակ և ճկուն խողովակով՝ բրոնխոսկոպով, հիվանդի  բերանից կամ քթից զգուշությամբ ներհրում է կատարում դեպի թոքերի օդատար խողովակներ՝ անցնելով կոկորդով, շնչափողով, գլխավոր բրոնխներից մեկնումեկով (աջ կամ ձախ) դեպի խոշոր և ավելի բարակ բրոնխներ։ Բրոնխոսկոպի ազատ ծայրին տեղադրված է լուսավորող տեսախցիկ, որը հնարավորություն է տալիս զննել ներսը։

Բրոնխոսկոպիայի արդյունքները բժշկին օգնում են, որ նա ընտրի պացիենտի թոքային խնդրի բուժման լավագույն տարբերակը։

Եթե պատկերային հետազոտություններ՝ ռենտգենոգրաֆիայի կամ համակարգչային շերտագրման արդյունքներում կան շեղումներ, բժիշկը կարող է ուղեգրել բրոնխոսկոպիայի՝ թոքերի օդատար խողովակների ներսը զննելու և ախտորոշումը ճշգրտելու համար։

Բրոնխոսկոպիա կարող է նշանակվել հետևյալ դեպքերում՝

թոքային հիվանդություններ, որոնք ընթանում են շնչառությանն ուղեկցող խզզոցներով, մշտական հազով կամ շնչարգելությամբ ու հևոցով,

թոքերի վարակային ախտահարումներ,

շնչուղիների և թոքերի ուռուցքային հիվանդություններ,

ջերմության բարձրացմամբ ընթացող շնչառական հիվանդություններ։

Բրոնխոսկոպիա կարող է նշանակվել նաև շնչուղիների տարբեր հիվանդությունների դեպքում, այդ թվում՝

քաղցկեղի բուժում,

շնչուղիներից օտար մարմինների հեռացում,

շնչուղիներից լորձի և հեղուկների կուտակումների հեռացում։

Բրոնխոսկոպիկ հետազոտություն անցնելու համար, մինչև բրոնխոսկոպիան, 6-12 ժամվա ընթացքում սնունդ չի կարելի ընդունել։

Եթե հիվանդը հավելյալ բուժում է ստանում, ապա պետք է խորհրդակցի բժշկի հետ՝ դեղամիջոցների ընդունման կարգի և հնարավոր փոփոխությունների մասին։

Բրոնխոսկոպիան սովորաբար կատարվում է թեթև սեդացիայի ներքո, երբ պացիենտն  արթուն է, սակայն զգում է որոշակի թուլություն ու քնկոտություն։

Հետազոտությունից առաջ բժիշկը տալիս է տեղային անզգայացնող դեղ՝ կոկորդի սփրեյ կամ ըմպանի ողողման լուծույթ։ Կարող են կատարվել նաև որոշակի ներարկումներ, որին կհաջորդի անզգայացնումն ու քնկոտությունը։

Մի քանի օրվա ընթացքում կոկորդում հնարավոր է զգալ որոշակի գրգռվածություն և ցավեր, որոնք սովորաբար մեկից երկու շաբաթվա ընթացքում աստիճանաբար անհետանում են։

Բրոնխոսկոպիան սովորաբար անվտանգ է, սակայն, ինչպես յուրաքանչյուր բժշկական միջոցառում, այն ևս կապված է որոշակի ռիսկերի հետ, այդ թվում՝

արյունահոսություն,,

վարակ,

բրոնխոսկոպիայից հետո՝ շնչառության որոշակի խանգարում։

Շատ հազվադեպ կարող է առաջանալ թոքի կոլապս։

Բոլոր այս բարդությունների դեպքում անհրաժեշտ է դիմել բժշկի խորհրդատվությանը, ով  կձեռնարկի համապատասխան բուժում։
